Liability Waiver

I realize that participation in dance and fitness classes and activities could involve some personal injury. I understand that it is advisable to obtain physician approval before beginning any new physical activity.   Despite precautions, accidents and injuries may occur.  By signing this release form, I (the dancer) assume all risks related to the use of any and all spaces used by Ballet Arts & Fitness LLC. 

 

I agree to release and hold harmless Ballet Arts & Fitness LLC including its teachers, dancers, staff members, and facilities used from any cause of action, claims, or demands now and in the future.  I will not hold Ballet Arts and Fitness LLC liable for any personal injury or any personal property damage, which may occur on the premises before, during, or after classes.  Further more, I agree to observe all facility rules and take full responsibility for my behavior in addition to any damage that I may cause to the facility utilized by Ballet Arts & Fitness LLC.

In addition, I agree to allow Ballet Arts & Fitness LLC permission to use photos of me from classes, performances and other studio events to promote the studio in social and traditional media.
